Case management services and representative payee program
Service Objective: The focus of case management is to serve vulnerable adults who have the potential of becoming victims of abuse, neglect, exploitation, self-neglect, and who are in need of supportive services to be self-sufficient toward maintaining their independence in the community.
Licensed social workers provide a variety of supportive services including but not limited to:
- Healthcare Management
- Advocacy / Counseling / Support
- Assessments of the individual’s overall needs
- Transportation / Escorts
- Financial Management
- Social Needs
- Crisis Intervention
The Representative Payee Program provides financial management to adults who are in need of a payee, or are required by the Social Security Administration office to have a payee. Additional services under this program include budgeting and paying bills to meet clients’ basic needs.
