You can apply by sending an application in to ESPregistration@lexingtonky.gov. Applications will be accepted by email only. All completed applications should be accompanied by a current immunization for each child.

Medical Forms
Immunization forms must be filled out by a medical professional and have an expiration date listed. Out-of-state immunizations may be accepted, but it may be required to schedule an appointment with your child's physician or KY Health Department to have it transferred to a Kentucky state form in order for your child to attend. Students with expired immunization forms will not be able to attend until we receive an updated form.
Scholarships
Parents or guardians must complete the form linked above and provide proof of eligibility to qualify. Military identification, KTAP, medical assistance cards, documentation of foster care or other proof of government assistance is acceptable as proof of eligibility.
