Form 211-2 Account Review
This form is also known as a “Cash Disbursement Request” and used as a cover sheet to request payment for a vendor that’s being paid on a Purchase Order or a one time individual payment. It has sections for the complete accounting chart fields and approvals.

Form 211-5 Disbursement Request
This form is also known as an “Account Review Form” and it’s used as an internal means to transfer funds between specific accounts or divisions.

Building Inspection

Application for Contractors Registration
This form is used to obtain general information from an applicant that is wishing to become a registered contractor in Lexington-Fayette County.

Kennel Zoning Approval
This form is used to obtain general information from an applicant that is wishing to operate a Kennel at a home, this form is required by the humane society, for zoning approval and to verify that the kennel will not be used for commercial purposes.

Kentucky Energy Conservation Code for Buildings Project Information Data
This is a work book to be filled out in addition to the Commercial application for applicants to obtain commercial permits that fall into the category that has this requirement.

Outdoor Seating Application
This form is used to obtain general information from an applicant that is wishing to obtain permission for outdoor seating at a restaurant.

Review Board
This form is used to inform the customer the applicable agencies that will either attend the review board meeting or the names and addresses of the agencies and reviewers, in the case that the customer would like to submit separately.

Wood Deck Guidelines
This form is used to inform the customer the applicable codes and guidelines for deck requirements.

Open Records Request
Used to request documents of public record.

Petition of Appellant
Used to appeal a notice or penalty issued by Code Enforcement.

Transfer of Ownership
Informs Code Enforcement of a new owner whom is aware of all outstanding code violations and has accepted the responsibility to make necessary corrections.

 

Community Development

Community-Wide Housing Emergency Repair Program
This program will provide financial assistance of $500 - $5,000 to low-moderate income owner-occupied homeowners with emergency repairs for electrical, plumbing, heating and air conditioning systems. You must be income eligible per federal guidelines. As of November 1, all heating emergency assistance will be referred to Community Action.

This form is used to obtain information from an applicant that is applying to make exterior modifications to a building and/or to a site on a property located within the Courthouse Area Design Overlay Zone. The information on the form will be used to initiate the design review process for approval by the Courthouse Area Design Review Board or the Design Review Officer.

Special Events Application
Anyone who plans to host a special event or festival in downtown Lexington must complete and submit a Special Event application to the Mayor’s Office of Special Events, 200 East Main Street, Lexington, KY 40507.  Application must include a $100 security deposit.

Street Banner Application
Anyone who would like to display banners for special events, conventions, etc. must complete and submit a street banner application to the Mayor’s Office of Special Events, 200 East Main Street, Lexington, KY  40507.  Application may be faxed to (859) 258-3194.

 

Parks & Recreation

Camp Scholarship
The LFUCG Division of Parks and Recreation provides for a 50% reduction in fees for families who qualify. Parents/Guardians must complete this scholarship form and provide proof of eligibility on an annual basis. Free lunch verification, KTAP, Medical Card, documentation of foster care or other proof of government assistance is acceptable as proof of eligibility. All forms must be submitted to the Division and verified before any scholarships are granted.

Physical & Verbal Altercation Policy

Request for Refund
Lexington Parks & Recreation will issue a 50% refund for program registration fees if a refund request form is submitted no fewer than seven (7) business days before the start of the activity; except in special circumstances such as medical reasons or summer school attendance.

This form is used to obtain general information from an applicant that is appealing to the Board of Adjustment for an Administrative Review.

Dimensional Variance Appeal
This form is used to obtain general information from an applicant that is appealing to the Board of Adjustment for a Dimensional Variance.

Major Subdivision & Development Plan Application
This application is used obtain basic information (mostly environmetnal) about requested subdivision and development plans that require Planning Commission approval.

Map Amendment Request Application (MAR)
This application is used to obtain general information from an applicant that is requesting a Map Amendment Request (MAR), also known as a zone change. A zone change, if approved, changes the zoning restrictions for a specified area, either a parcel, group of parcels or a portion of a parcel.

Zoning Ordinance Text Amendment Application (ZOTA)
This application is used obtain information about a requested amendment to the Zoning Ordinance. A text amendment can create new or alter existing permitted uses or requirements.  

 

Police

Analysis Request
This form may be used to request crime or traffic data for a specified time and location. Costs may be incurred in accordance with KRS 61.874

Civilian Traffic Collision Report
This form may be utilized to report a traffic collision in accordance with KRS 189.635(2) (4).

Position Interest Request
This form indicates an interest in a position and is completed prior to completing an application for employment as a police officer. The form includes an essential job elements section which ensures compliance with federal employment guidelines.

 

Purchase of Development Rights

PDR Donation
This form is used to obtain information from an applicant that is applying to donate a conservation easement on their farm. The information on the form will be used to evaluate the farm with the criteria for accepting a donated conservation easement.

Round 8 - 2009 Application
This form is used to obtain information from an applicant that is applying to sell a conservation easement on their farm. The information on the form will be used to evaluate the ranking of the farm for conservation easement acquisition.

Waste Management

 

Exemption Request Form
LFUCG trash/recycling customers who have a physical or medical limitation that prohibits them from rolling the carts to the curb for collection.

Water Quality

Application for Sewer User Charge Exclusion
Commercial and industrial facilities (Schedule B Users) that pay sewer user fees for water which is not discharged to the LFUCG sewer system may receive a credit for fees paid. Before the facility can submit this application for credit, the facility must have a meter installed to measure water that is not discharged to the sewer, and this meter must be inspected and pre-approved by an Environmental Inspector. For more information about qualifying for this program, please contact Jennifer Myatt (859-425-2409) at the Division of Water Quality, Compliance and Monitoring Section.

Grease Interceptor Cleaning and Maintenance
Food Service Establishments (FSEs) are required by Lexington-Fayette County Ordinance to document cleanings of their grease removal equipment. This form must be kept on site and be made available to Environmental Inspectors during an inspection.
